علاج البرد مضاد الاحتقان بالفم مع مضاد الحساسية

(اوصف صح) خطأ في علاج نزلات البرد⛔

طبعا كلنا متفقين أن علاج نزلات البرد هو symptomatic treatment 

ومن أفضل الأدوية اللي بتخفف أعراض البرد والزكام هي ال

Oral decongestant plus antihistamine

👎شفت البعض بيطلع مكان ده ال 

Normal saline + antihistamine 

⛔ وده خطأ شائع مينفعش

الoral decongestants عامة وال pseudoephedrine خاصة هي أفضل علاج للاحتقان وهي ال drug of choice ومحلول الملح مع مضادات الهستامين مفعوله أضعف منها بكتير 😱

✔والكلام ده من uptodate بيقول👇:

● The combination of antihistamines and decongestants may be more beneficial than either component alone. Antihistamine use alone in patients with the common cold, however, is of minimal benefit and frequently results in troublesome side effects. 

● Saline nasal sprays may help nasal symptoms of the common cold. A 2015 systematic review of saline nasal irrigation for acute upper respiratory infections concluded there may be symptomatic benefits, but there was limited evidence to support this conclusion, as the available trials were small and had a high risk of bias

🔵 لذلك فالاعتماد على محلول الملح فقط لوحده أو مع مضاد الهستامين مش صح ومش هيجيب معاك نفس نتيجة ال  oral decongestant plus antihistamine لكن ممكن تستخدم محلول الملح كإضافه مع ال combination اللي قلنا عليه مفيش مشكلة.

 👍طيب امتى لانستخدم ال oral decongestant plus antihistamine ولازم نستخدم حاجة للحساسية مع محلول ملح؟

في الحالات اللي مينفعش يؤخذ فيها مضاد للاحتقان oral decongestant

✔زي ايه مثلا(على سبيل المثال لا الحصر)؟

واحدة حامل pregnancy أو بترضع lactation 

مريض قلب 

مريض ضغط خاصة اللي ضغطه مش منضبط

مريض مضادات الاحتقان بتعمل له أرق 

وهكذا 

✔وبرضه الكلام ده من uptodate 👇

They may be reasonable options in some patients who are not able to tolerate the more effective therapies.

✔الأصل والصح هو إعطاء oral decongestant + antihistamine ولاتلجأ للحل الآخر إلا في حال أن الحاجات دي متنفعش
